I'm a clinical pharmacist and faculty member who has been embedded in primary care and family medicine residency training since 2012. For years, I ran a weekly Medical Decision Making education series, structured sessions designed to help residents think more rigorously about the evidence behind clinical choices. I've sat through more journal clubs than I can count. Most of them were a chore. A resident spent hours building slides, the group half-listened, an attending asked a few pointed questions, and everyone walked out having caught up on their charting. Engagement was the problem and I think I figured out a way to change that. Dispense As Written is my attempt to fix it. This is an independent project, no institution, no sponsor, no agenda beyond the evidence. Every episode is a rigorous, clinical trial breakdown built around one question: does this change what you do for your patient? Not surrogate endpoints. Not disease-oriented data dressed up as outcomes. Patient-oriented evidence, the kind that actually matters at the bedside. Beyond individual trials, I also tackle the fundamentals of evidence-based medicine, trial designs, levels of evidence, and searching strategies, not as abstract concepts, but grounded in real studies so the methodology always connects back to a clinical question you're actually facing. Meet the Host
GREGORY CASTELLI
Gregory B. Castelli, PharmD, FCCP, BCPS, BC-ADM, CDCES is a pharmacist and Associate Professor in the Department of Family Medicine at the University of Pittsburgh School of Medicine where he serves as the Director of Academic and Clinical Pharmacy. He is also the Interprofessional Education Thread Co-Lead in the new Three Rivers Curriculum.
Dr. Castelli received the PharmD degree from Wilkes University and then completed a PGY1 Pharmacy Residency, PGY2 Ambulatory Care Residency, and Faculty Development Fellowship at UPMC St. Margaret. Following residency, he was a Clinical Assistant Professor at West Virginia University Schools of Pharmacy and Medicine where he established a clinical practice site with the WVU Department of Family Medicine. Dr. Castelli returned to Pittsburgh to join the UPMC St. Margaret Family Medicine and Pharmacy Residency programs from 2016-2023. Here, Dr. Castelli served as the Director of the PGY1 Pharmacy Residency, Associate Director of Faculty Development Fellowship, and Director of Medical Decision Making Education.
Dr. Castelli’s practice is in primary care. His interests include evidence-based medicine and management of chronic conditions including diabetes and asthma/COPD. Dr. Castelli is active in ACCP and STFM national organizations.
