The Disclosure of Relevant Financial Relationships

Because Pinnacle is responsible for upholding the Standards for Integrity and Independence in Accredited Continuing Education, please disclose all financial relationships that you have had with ineligible companies in the 24 months prior to this education’s start date. For each relationship, enter the name of the ineligible company and the nature of the financial relationship(s).
There is no minimum financial threshold; we ask you to disclose all financial relationships, regardless of the amount, with ineligible companies. You should disclose all financial relationships regardless of the potential relevance to the educational activity you are involved in.
Examples of financial relationships include employee, researcher, consultant, advisor, speaker, independent contractor (including contracted research), royalties or patent beneficiary, executive role, and ownership interest. Individual stocks and stock options should be disclosed; diversified mutual funds do not need to be disclosed. Research funding from ineligible companies should be disclosed by the principal or named investigator even if that individual’s institution receives the research grant and manages the funds.
An ineligible company is any entity whose primary business is producing, marketing, selling, re-selling, or distributing healthcare products used by or on patients.

Speaker agrees to indemnify, defend, and hold harmless Pinnacle, including its officers, agents, owners, employees, and contractors, for any acts or omissions of Speaker committed during the entire conference period. This indemnification extends to any claims asserted by third parties for damages sustained as a result of any act or failure to act by the Speaker.
Speaker agrees to not disparage Pinnacle (which includes its owners, officers, and conference sponsors and participants), including making any written or oral statements, including social media posts or online reviews, that may portray Pinnacle in a negative light.
This presentation is considered a “work for hire,” where all intellectual property rights attached to the presentation, including presentation slides, exhibits, handouts, or recordings (the “Components”) belong to Pinnacle Conference, LLC. Speaker agrees to execute any necessary paperwork to memorialize Pinnacle Conference’s ownership interest. To the extent the presentation and Components may not be considered a “work for hire,” Speaker confers Pinnacle Conference, LLC a perpetual, royalty-free license to use, replicate, and publish the presentation and Components, including the right to create derivative works. Pinnacle Conference’s license permits it to retain any revenue generated from the use of the presentation or its Components or the creation of any derivative work. Speaker agrees to execute any documents necessary to memorialize this license.
